The unique context of the rural setting provides special challenges to furnishing ethical healthcare to its approximately 62 million inhabitants. Although rural communities are widely diverse, most have the following common features: limited economic resources, shared values, reduced health status, limited availability of and accessibility to healthcare services, overlapping professional–patient relationships and care giver stress. These rural features shape common healthcare ethical issues, including threats to confidentiality, boundary issues, professional–patient relationship and allocation of resources. To date, there exists a limited (...) focus on rural healthcare ethics shown by the scarcity of rural healthcare ethics literature, rural ethics committees, rural focused ethics training and research on rural ethics issues. An interdisciplinary group of rural healthcare ethicists with backgrounds in medicine, nursing and philosophy was convened to explore the need for a rural healthcare ethics agenda. At the meeting, the Coalition for Rural Health Care Ethics agreed to a definition of rural healthcare ethics and a broad-ranging rural ethics agenda with the ultimate goal of enhancing the quality of patient care in rural America. The proposed agenda calls for increasing awareness and understanding of rural healthcare ethics through the development of evidence—informed, rural-attuned research, scholarship and education in collaboration with rural healthcare professionals, healthcare institutions and the diverse rural population. Objective To assess parental permission for a neonate's research participation using the MacArthur competence assessment tool for clinical research (MacCAT-CR), specifically testing the components of understanding, appreciation, reasoning and choice. Study Design Quantitative interviews using study-specific MacCAT-CR tools. Hypothesis Parents of critically ill newborns would produce comparable MacCAT-CR scores to healthy adult controls despite the emotional stress of an infant with critical heart disease or the urgency of surgery. Parents of infants diagnosed prenatally would have higher MacCAT-CR scores than parents (...) of infants diagnosed postnatally. There would be no difference in MacCAT-CR scores between parents with respect to gender or whether they did or did not permit research participation. Participants Parents of neonates undergoing cardiac surgery who had made decisions about research participation before their neonate's surgery. Methods The MacCAT-CR. Results 35 parents (18 mothers; 17 fathers) of 24 neonates completed 55 interviews for one or more of three studies. Total scores: magnetic resonance imaging (mean 36.6, SD 7.71), genetics (mean 38.8, SD 3.44), heart rate variability (mean 37.7, SD 3.30). Parents generally scored higher than published subject populations and were comparable to published control populations with some exceptions. Conclusions The MacCAT-CR can be used to assess parental permission for neonatal research participation. Despite the stress of a critically ill neonate requiring surgery, parents were able to understand study-specific information and make informed decisions to permit their neonate's participation. Our objective is to understand how parents and children perceive their roles in decision making about research participation. Forty-five children (ages 4-15 years) with or without a chronic condition and 21 parents were the participants. A semistructured interview assessed perceptions of up to 4 hypothetical research scenarios with varying levels of risk, benefit, and complexity. Children were also administered the Peabody Picture Vocabulary Test, Third Edition, to assess verbal ability, as a proxy for the child's cognitive development. The audiotaped interviews (...) were transcribed and analyzed for themes related to parent and child decision-making roles. Both parents and children varied in their perceptions of decision-making roles. Child perceptions of parental influence on decision making as knowledge-based increased with cognitive development, whereas perceptions of parental influence as power-based decreased. Both children and parents commented that they would collaborate with each other when making decisions. Collaborative decision making appeared to increase with cognitive development. These findings suggest that approaches to child assent and parent permission should consider the parent-child relationship and how children and families typically make decisions. Future research is necessary to explain variation in the process of research decision making across children and families, explore the role of collaboration on children's decision-making skills, and understand developmental trajectories and mechanisms related to research decision making. Objective: To determine whether bioethicists are distributed along a rural-to-urban continuum in a way that reflects potential need of those resources as determined by the general population, hospital facilities and hospital beds.Methods: US members of a large, multidisciplinary professional society, the American Society of Bioethics and Humanities , the US population, hospital facilities and hospital beds were classified across a four-tier rural-to-urban continuum. The proportion of each group in rural settings was compared with that in urban settings, and odds ratios (...) were calculated with 95% confidence intervals.Results: Although 91% of ASBH members live or work in urban settings, only 66% of the US population did so. In contrast, 2% of ASBH members live or work in rural settings compared with 13% of the population. ASBH members were 10.7 times as likely to be represented in urban than in rural settings when compared with the general population, 25.6 times and 6.9 times as likely with regard to hospital facilities and hospital beds, respectively.Conclusions: Using various comparisons it was found that ASBH members are under-represented in rural as compared with urban settings. Although not all bioethicists are ASBH members, these findings suggest that the availability of professional bioethical resources may be inadequate in rural America. The disparities that were found may have considerable effect on ethics scholarship, research, ethical committees and education, and adds to the argument that rural American communities are under-served. Somatic or inherited mutations in the adenomatous polyposis coli (APC) gene are a frequent cause of colorectal cancer in humans. APC protein has an important tumor suppression function to reduce cellular levels of the signaling protein β‐catenin and, thereby, inhibit β‐catenin and T‐cell‐factor‐mediated gene expression. In addition, APC protein binds to microtubules in vertebrate cells and localizes to actin‐rich adherens junctions in epithelial cells of the fruit fly Drosophila (Fig. 1). We hypothesize that aspects of animal multicellularity originated before the divergence of metazoans from fungi and social amoebae. Polarized epithelial tissues are a defining feature of metazoans and contribute to the diversity of animal body plans. The recent finding of a polarized epithelium in the non‐metazoan social amoeba Dictyostelium discoideum demonstrates that epithelial tissue is not a unique feature of metazoans, and challenges the traditional paradigm that multicellularity evolved independently in social amoebae and metazoans. An alternative view, presented here, is (...) that the common ancestor of social amoebae, fungi, and animals spent a portion of its life cycle in a multicellular state and possessed molecular machinery necessary for forming an epithelial tissue. Some descendants of this ancestor retained multicellularity, while others reverted to unicellularity. Beta‐catenin is a multifunctional protein with critical roles in cell‐cell adhesion, Wnt‐signaling and the centrosome cycle. Whereas the roles of β‐catenin in cell‐cell adhesion and Wnt‐signaling have been studied extensively, the mechanism(s) involving β‐catenin in centrosome functions are poorly understood. β‐Catenin localizes to centrosomes and promotes mitotic progression. NIMA‐related protein kinase 2 (Nek2), which stimulates centrosome separation, binds to and phosphorylates β‐catenin. β‐Catenin interacting proteins involved in Wnt signaling such as adenomatous polyposis coli, Axin, and GSK3β, are also localized at (...) centrosomes and play roles in promoting mitotic progression. Additionally, proteins associated with cell‐cell adhesion sites, such as dynein, regulate mitotic spindle positioning. These roles of proteins at the cell cortex and Wnt signaling that involve β‐catenin indicate a cross‐talk between different sub‐cellular sites in the cell at mitosis, and that different pools of β‐catenin may co‐ordinate centrosome functions and cell cycle progression. Integrins are ubiquitous trans‐membrane adhesion molecules that mediate the interaction of cells with the extracellular matrix (ECM). Integrins link cells to the ECM by interacting with the cell cytoskeleton. In cases such as leukocyte binding, integrins mediate cell‐cell interactions and cell‐ECM interactions. Recent research indicates that integrins also function as signal transduction receptors, triggering a number of intracellular signaling pathways that regulate cell behavior and development. A number of integrins are known to stimulate changes in intracellular calcium levels, resulting in (...) integrin activation. Although changes in intracellular calcium regulate a vast number of cellular functions, this review will discuss the stimulation of calcium signaling by integrins and the role of intracellular calcium in the regulation of integrin‐mediated adhesion. Kant informs us that the categorical imperative is a synthetic a priori proposition. It might be thought that Kant means thereby to say that we have an a priori rational insight, somewhat like that which Plato claimed for the Forms, into this basic moral truth. W. D. Ross had a similar view about human rational insight into certain basic moral principles, and in his book Kant's Ethical Theory he attritubes such a view to Kant.I argue that this initially plausible interpretation (...) is not correct, and that the "rational intuitionism" that it involves goes against Kant's whole approach to philosophy, in the first Critique as well as in the moral philosophy. I develop a new alternative interpretation, as follows: the categorical imperative is synthetic because it contains a claim about the causal/motivational power we have to act purely from the motive of duty, without any assistance from "sensible" or empirical motives, such as pleasure or pain. This claim, qua motivational, goes beyond the meaning of the categorical imperative considered as a mere ethical precept. It is a priori because this motive's source is the noumenal self.Hence to justify the categorical imperative is to show that this sort of non-sensible motive is or could be active in us as agents, and Kant is an "internalist": one who believes that morality and motivation are internally, analytically connected. Nach Kant ist der kategorische Imperativ ein synthetischer Satz a priori. As one of the preeminent philosophers of the twentieth century, W. V. Quine made groundbreaking contributions to the philosophy of science, mathematical logic, and the philosophy of language. This collection of essays examines Quine's views, particularly his holism and naturalism, for their value to feminist theorizing today. Some contributors to this volume see Quine as severely challenging basic tenets of the logico-empiricist tradition in the philosophy of science—the analytic/synthetic distinction, verificationism, foundationalism—and accept various of his positions as potential resources for (...) feminist critique. Other contributors regard Quine as an unrepentant empiricist and, unlike feminists who seek to use or extend his arguments, they interpret his positions as far less radical and more problematic. In particular, critics and advocates of Quine's arguments that the philosophy of science should be "naturalized"—understood and pursued as an enterprise continuous with the sciences proper—disagree deeply about whether such a naturalized philosophy is "philosophy enough." Central issues at stake in these disagreements reflect current questions of special interest to feminists and also bridge the analytic and postmodern traditions. Appeals to some thesis of underdetermination, to the idea that scientific theories and hypotheses are not entailed by the evidence that supports them, are common in feminist philosophy of science. These appeals seek to understand and explain how androcentrism and other problematic approaches to gender have found their way into good science, as well as the reverse – how feminist approaches to gender and science that are also value-laden, can contribute to good science. Focusing on W.V. Quine’s positions on holism (...) and underdetermination, I argue that although Quine’s general thesis of holism does not entail his thesis of global underdetermination, his account of and arguments for moderate holism entail a localized and generalizable underdetermination, which I call moderate underdetermination. This moderate underdetermination is not transient or true of only some theories and hypotheses. Rather, it is a permanent state of affairs that characterizes all reasonably inclusive theories and hypotheses, however much evidence for them accumulates. I contend that Quine’s moderate holism entails the most robust and defensible argument for localized underdetermination, and that feminists would do well to appeal to it in arguing that values inform good science. Cambridge, Cambridge University Press, 1999. Pp. xxiv + 436. Cloth, $54.95. This book by Allen Wood in its first half gives us a state-of-the-art survey of traditional topics in the interpretation of Kant's ethics, and in the second half breaks new ground, and significantly widens the canon of works that (...) need to be taken account of in presenting Kant's ethics as a whole. Wood tells us that his main criticism of previous interpretations of Kant's ethics is that they overemphasize the universal law formulation of the categorical imperative, when the most promising version is the formula of humanity. One of the major contributions of this book is the detailed argument (passim, but especially Chapter IV) that the formula of humanity is the preferable one for understanding Kant's ethics in application, and the one used most in the Metaphysics of Morals.Part I takes the reader on the familiar traversal of the text of Kant's Groundwork of the Metaphysics of Morals. Chapter 1 is a discussion of the topics that come up in Chapter One of the Groundwork. Chapter 2 is a coverage of the first part of Kant's Chapter Two. Chapters 3, 4, and 5, are discussions of what are usually called the first (universal law), second (humanity), and third (autonomy and kingdom of ends) formulations of the categorical imperative. The discussion of Kant's Chapter Three is brief (basically, Wood's pages 171-182).Wood comments that the proper way to expound Kant's views on ethics would be, after (1) an introductory exposition based in the Groundwork, (i.e., his Chapters 1-5) to present (2) Kant's views on "human nature and its consequences for the application of the foundational principles" (13) (his Chapters 6-9). Only then would one be in a position to "provide (3) a systematic exposition of the Metaphysics of Morals as the definitive form of Kant's practical philosophy" (13). But such an undertaking would have been too ambitious for any single book. Hence Wood's book omits part (3), though he makes significant use of the Metaphysics of Morals throughout.Wood urges his readers not to neglect his extensive endnotes (xv), where sometimes [End Page 151] important matters are discussed that could not be accommodated within the main text. In addition, in the notes one can find some fascinating passages, e.g., (1) a significant self-criticism (345, n.12). (2) Another note finds Wood saying that Andrea Dworkin borrows her views of sexuality from Simone de Beauvoir (without acknowledgment), and that Beauvoir and Sartre's views were largely anticipated by Kant. Then he adds this zinger: "These comparisons also shed light on the cogency of the condescending explanations usually offered for Kant's views on sex. For it would not naturally occur to us to attribute the views of Beauvoir or Sartre to their sexual inexperience, to the lingering effects of their excessively religious upbringing or to the puritanical attitudes characteristic of mid-century Parisian intellectuals" (397, n.11).One of the traditional topics Wood discusses is the proper formulation of maxims for evaluation by the universal law formulation. He thinks that there are bound to be some systematic failures to achieve correct results because of inadequacies of this formulation, but he discusses fairly extensively how to minimize such erroneous outcomes by correct formulations of maxims (97-107), and his discussion makes some useful contributions to this once heavily discussed topic. One might add another, almost commonsense suggestion to his: Maxims mention ends of action, as well as proposed action-kinds; since actions are thus described within maxims as means to ends, action-descriptions would include only action characteristics that are causally relevant to the achievement of the ends of actions. This reminder enables us to eliminate many irrelevant action-descriptors.Part II of the Wood book is entitled "Anthropological Applications." I consider the issues that recent feminist critiques of science raise for contemporary empiricist philosophy of science. Three particular focuses of feminist criticism are addressed: the social arrangements within and outside science communities that divide cognitive labor and authority, the apparent androcentrism in several of the social and biological sciences, and the use of models that reflect Western political experience in the biological sciences. ;I urge that a consideration of these issues indicate that science communities interact with our larger society (...) with consequences for the directions of scientific research, and with consequences for our common sense understandings of human and nonhuman nature. In the light of feminist criticisms, I re-examine the philosophy of science of a major American philosopher, W. V. O. Quine. The interplay between Quine's work and feminist scholarship provides support for two thesis. ;First, I urge that contemporary philosophy of science has too narrowly construed the factors relevant to the epistemology of science. Among the factors that feminists have shown to have an impact on research programs and theoretical frameworks are Western gender arrangements and Western political experience generally. Much contemporary philosophy of science lacks the categories that would enable these factors to be incorporated in the epistemology of science. I argue that frameworks need to be developed that can both accommodate these factors and account for the success of our theorizing. ;Secondly, I urge that the thesis of "epistemological individualism" be abandoned by empiricists. I argue that the feminist criticisms considered and aspects of Quine's philosophy of science indicate that the thesis is empirically unsound. I argue that the acquirers of knowledge are communities, and not individuals, and I use aspects of Quine's philosophy of science and the current presence of androcentric and feminist perspectives in the sciences to support this view.
